Les personnes touch\u00e9es doivent prendre de l\u2019insuline synth\u00e9tique pour rester en bonne sant\u00e9. Comme le diab\u00e8te de type 1 se d\u00e9clare surtout dans l\u2019enfance et l\u2019adolescence, les besoins en m\u00e9dicaments faciles \u00e0 prendre sont grands.<\/p>\n\n\n\n<p><strong>Insuline par voie orale<\/strong><\/p>\n\n\n\n<p>Actuellement, l\u2019insuline est essentiellement administr\u00e9e par injections, plus rarement sous forme \u00e0 inhaler. Il n\u2019a pas encore \u00e9t\u00e9 possible de fabriquer de l\u2019insuline sous forme de comprim\u00e9s, car l\u2019insuline est d\u00e9truite dans le syst\u00e8me digestif avant de pouvoir d\u00e9ployer son effet. \u00c0 l\u2019universit\u00e9 de Sydney en Australie, une \u00e9quipe de recherche a d\u00e9velopp\u00e9 une forme d\u2019insuline d\u2019un nouveau genre&nbsp;: elle se compose d\u2019infimes particules appel\u00e9es nanoparticules. Celles-ci prot\u00e8gent l\u2019insuline de l\u2019acide gastrique que produit l\u2019estomac et qui la d\u00e9truirait, de sorte que l\u2019insuline peut atteindre l\u2019intestin et passer ensuite dans l\u2019organisme.<\/p>\n\n\n\n<p><\/p>\n<\/div>\n\n\n\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\">\n<figure class=\"wp-block-image size-large is-resized\" id=\"folge2\"><img decoding=\"async\" src=\"https:\/\/animalwelfare.interpharma.ch\/wp-content\/uploads\/sites\/5\/2025\/08\/20250806_iph_grafiken_animal_welfare_grafik_Diabetes_FR-721x1024.jpg\" alt=\"\" class=\"wp-image-11262\" style=\"width:523px;height:auto\"\/><\/figure>\n\n\n\n<p class=\"has-text-align-left\" id=\"folge2\">Source: <a href=\"https:\/\/www.aok.de\/pk\/chronische-erkrankungen\/diabetes\/\" target=\"_blank\" rel=\"noreferrer noopener\">AOK<\/a>, diabetesuisse.ch, notre r\u00e9sum\u00e9<\/p>\n<\/div>\n<\/div>\n\n\n\n<p><strong>Des m\u00e9thodes de substitution jouent un r\u00f4le important dans le d\u00e9veloppement<\/strong><\/p>\n\n\n\n<p>Cette nouvelle forme d\u2019administration par voie orale a \u00e9t\u00e9 test\u00e9e, tant par des m\u00e9thodes de substitution que dans des exp\u00e9riences sur animaux. Aux phases pr\u00e9coces de la recherche, les scientifiques ont utilis\u00e9 des mod\u00e8les in vitro, en particulier des tissus d\u2019intestins humains, pour tester la capacit\u00e9 d\u2019absorption des nanoparticules. Ces tests ont montr\u00e9 une am\u00e9lioration significative de l\u2019absorption de l\u2019insuline par rapport aux insulines classiques. En compl\u00e9ment, des organismes mod\u00e8les de substitution comme le n\u00e9matode Caenorhabditis elegans ont \u00e9t\u00e9 utilis\u00e9s pour \u00e9tudier de plus pr\u00e8s les effets. Cependant, pour \u00e9valuer en d\u00e9tail l\u2019efficacit\u00e9 de la nouvelle forme d\u2019insuline, il a fallu faire <br>d&rsquo;autres exp\u00e9riences sur des animaux. L\u2019\u00e9quipe de recherche a effectu\u00e9 des exp\u00e9riences sur la souris et le singe pour examiner l\u2019effet de baisse de la glyc\u00e9mie, le risque d\u2019hypoglyc\u00e9mie et d\u2019autres effets secondaires \u00e9ventuels. Bilan: la glyc\u00e9mie des animaux a diminu\u00e9 comme souhait\u00e9 et il n\u2019y a pas eu d\u2019effets secondaires n\u00e9fastes. Ceci est un progr\u00e8s consid\u00e9rable, car jusqu\u2019\u00e0 pr\u00e9sent, on consid\u00e9rait comme quasiment impossible d\u2019assurer l\u2019efficacit\u00e9 de l\u2019insuline sous forme de comprim\u00e9s. Si cette efficacit\u00e9 se confirme dans d\u2019autres \u00e9tudes et chez l\u2019\u00eatre humain, des millions de personnes atteintes de diab\u00e8te dans le monde entier pourraient un jour se passer des injections d\u2019insuline qui ne sont pas sans poser des probl\u00e8mes.<\/p>\n\n\n\n<p><strong>Un m\u00e9dicament qui emp\u00eache le diab\u00e8te?<\/strong><\/p>\n\n\n\n<p>\u00c0 la Johns Hopkins University de Baltimore (\u00c9tats-Unis), une \u00e9quipe de recherche travaille sur une autre approche contre le diab\u00e8te: il s\u2019agit d\u2019un m\u00e9dicament qui combat directement la cause du diab\u00e8te de type&nbsp;1. Les scientifiques ont d\u00e9velopp\u00e9 ce que l\u2019on appelle un anticorps monoclonal, une substance active qui reconna\u00eet et influence de mani\u00e8re cibl\u00e9e certaines cellules de l\u2019organisme. Dans ce cas, l\u2019anticorps se lie aux cellules b\u00eata du pancr\u00e9as, c\u2019est-\u00e0-dire celles qui produisent l\u2019insuline mais, dans le diab\u00e8te de type&nbsp;1, sont d\u00e9truites par le syst\u00e8me immunitaire. Ce nouveau m\u00e9dicament prot\u00e8ge les cellules b\u00eata de cette attaque. L\u00e0 aussi, pour des raisons de s\u00e9curit\u00e9, il n\u2019a pas \u00e9t\u00e9 possible de se passer d\u2019exp\u00e9riences sur animaux pour tester l\u2019efficacit\u00e9 du traitement. Elles ont eu lieu chez la souris, \u00e0 haut risque de diab\u00e8te de type&nbsp;1. Or, il est apparu que les souris qui avaient re\u00e7u le m\u00e9dicament ne d\u00e9veloppaient pas la maladie et vivaient m\u00eame plus vieilles que les animaux non trait\u00e9s.<\/p>\n\n\n\n<p>Les tests sur l\u2019animal, actuellement encore indispensables, nourrissent l\u2019espoir de pouvoir un jour, gr\u00e2ce \u00e0 cette substance, gu\u00e9rir le diab\u00e8te, voire l\u2019emp\u00eacher de se d\u00e9clarer. Mais le d\u00e9veloppement du m\u00e9dicament n\u2019est pas encore suffisant pour pouvoir le tester chez l\u2019\u00eatre humain. L\u2019\u00e9quipe de recherche travaille actuellement \u00e0 une version \u00abhumanis\u00e9e\u00bb de l\u2019anticorps, c\u2019est-\u00e0-dire une substance active sp\u00e9cialement con\u00e7ue pour l\u2019organisme humain.<\/p>\n\n\n\n<p><strong>Pourquoi les m\u00e9thodes de substitution ne peuvent-elles pas (encore) remplacer enti\u00e8rement l\u2019exp\u00e9rimentation animale?<\/strong><\/p>\n\n\n\n<p>Les deux \u00e9tudes montrent qu\u2019en plus des m\u00e9thodes de substitution, les exp\u00e9riences sur animaux restent \u00e0 ce jour indispensables aux progr\u00e8s m\u00e9dicaux. Elles aident \u00e0 tester la s\u00e9curit\u00e9 et l\u2019efficacit\u00e9 de nouveaux traitements avant de pouvoir les employer dans le syst\u00e8me biologique complexe de l\u2019\u00eatre humain.
